Telehealth clinical decision-making requires nurse practitioners to adapt traditional assessment skills to a virtual environment by strengthening communication, observation, and technology-related competencies. Successful telehealth practice depends on accurately interpreting verbal and non-verbal patient cues, asking focused questions, understanding technology limitations, and using evidence-based strategies to support safe and effective patient outcomes.
Telehealth is a broad healthcare delivery approach that includes medical care, therapy, education, and remote monitoring through digital communication technologies. One important area of telehealth is telenursing, which allows nurses and advanced practice providers to assess, educate, and support patients remotely.
Telehealth encounters are generally categorized into two models:
Synchronous telehealth:Â Real-time communication between patients and clinicians through video or audio platforms.
Asynchronous telehealth:Â The exchange of patient information, such as images, videos, text messages, or recorded data, that is reviewed by clinicians at a later time.
For nurse practitioners, telehealth requires a shift in clinical assessment because many traditional physical examination techniques are limited in a virtual setting.
Clinical judgment in telehealth involves creating a mental picture of the patient by analyzing available information, including spoken communication, appearance, behaviors, and other observable cues. Research suggests that accuracy in telehealth clinical decision-making is influenced by the clinician’s experience, training, and ability to interpret limited information effectively (Zhang et al., 2024).
Before independently providing telehealth care, nurse practitioners should develop additional skills through education, practice, and preparation. These skills help ensure that virtual assessments remain safe, accurate, and patient-centered.
Important areas of preparation include:
Understanding personal clinical decision-making processes during virtual encounters.
Developing strategies for collecting complete patient histories.
Recognizing subtle changes in patient communication and behavior.
Learning how to identify situations that require an in-person evaluation.
Effective communication becomes even more important when clinicians cannot perform a complete physical examination. Nurse practitioners must rely heavily on active listening and careful observation to identify potential health concerns.
During telehealth visits, clinicians should focus on:
Asking additional clarifying questions to gather complete information.
Paying attention to the patient’s tone, speech patterns, and emotional responses.
Assessing breathing patterns, facial expressions, and other visible behaviors.
Evaluating the patient’s health literacy and the ability of caregivers to provide support when necessary.
Studies indicate that clinicians may need to intentionally adjust their communication techniques during telehealth visits because important non-verbal information can be more difficult to recognize compared with in-person encounters (Ford & Reuber, 2024).
A challenge of telehealth is the inability to observe the patient’s entire body and perform a hands-on physical examination. Because of these limitations, nurse practitioners must use available visual information strategically.
Helpful approaches include:
Maintaining eye contact by looking toward the camera.
Observing upper-body movements and facial expressions.
Encouraging patients to describe symptoms in detail.
Using video, images, or screen-sharing tools for patient education.
Reflecting back information to confirm understanding.
Engaging patients actively during virtual visits helps create a stronger provider-patient relationship and improves communication despite the physical distance between the clinician and patient (Wright et al., 2022).
Technology plays an essential role in telehealth, but technical limitations can interfere with accurate clinical decision-making. Poor camera quality, internet connectivity problems, audio issues, or software failures may reduce the amount of information available to the clinician (Zhang et al., 2024).
Technology-related barriers may include:
Limited access to reliable internet services.
Lack of high-quality communication devices.
Difficulty using digital healthcare platforms.
Reduced ability to observe important patient information.
These challenges may contribute to health disparities because patients with limited access to technology may not receive the same quality of virtual care as individuals with better digital resources (Ford & Reuber, 2024).
Clinical decision-making in telehealth is a complex reasoning process influenced by multiple factors. Nurse practitioners must consider not only patient symptoms but also the environment in which care is delivered.

Patient factors:Â Ability to communicate symptoms, health literacy, and access to support systems.

By recognizing these influences, nurse practitioners can develop safer approaches to virtual care and improve patient outcomes.
Telehealth requires nurse practitioners to expand traditional clinical assessment skills by focusing on communication, observation, and technology management. Although virtual care presents challenges, appropriate training and experience can improve clinical judgment and patient safety. By strengthening listening skills, recognizing non-verbal cues, and addressing technology barriers, nurse practitioners can provide effective and patient-centered telehealth services.
