In such a situation, as the substitute clinical educator, my main priority would be to ensure patient safety by preventing any further actions that might compromise it. To address the issue, I would privately meet with the nurse who is not performing adequately to discuss her actions, clinical objectives, and expected outcomes based on the necessary knowledge and skills. I would inquire about her approach to patient care in order to better understand her knowledge, strengths, weaknesses, and areas for improvement. Based on my observations, I would point out her mistakes and attempt to understand the reasons behind them. I would clearly explain the deficits in her knowledge, attitudes, and skills as observed in her clinical practice. The next step would be to set individualized learning expectations regarding her knowledge and skills during this private meeting.

Furthermore, I would identify the specific actions required for the nurse to succeed and prevent repeating the same mistakes in the future. We would document specific examples of unsafe actions, such as contaminating a dressing change for a central line, and missed actions, such as forgetting to perform a glucose monitor test. Following this, I would discuss the possibility of placing her on a learning probation or contract to ensure clear and specific expectations for her learning and patient care outcomes. If we mutually agree, I would formalize this in a written document, which we would both sign. This learning contract would allow the nurse to improve her ability to provide safe patient care. The contract would outline the learner’s specific knowledge, attitude, and skills (KAS), the deadline for successfully demonstrating these KAS, and the consequences if the nurse fails to meet the expectations by the given deadline (Oermann & Gaberson, 2017). During the contract period, I would regularly meet with the nurse to assess her progress and provide feedback based on the demonstration of expected outcomes.
