Additionally, your work should align closely with the provided rubric, as it outlines the specific criteria on which your submission will be evaluated.

Requirement A: Clinical Practice Experience Record

What must be included in the Clinical Practice Experience (CPE) Record submission?

You are required to submit a completed and signed CPE Record as a separate attachment, distinct from your e-portfolio. The form should contain your first and last name, the submission date, and your course instructor’s name. It must also list the completion date for every activity documented and include a typed signature confirming that all activities for Phase 1 and Phase 2 have been completed.

Requirement B: E-Portfolio Deliverables

Evaluation Rubric
