This course introduces students to foundational and applied concepts in community and population health nursing. Emphasis is placed on the social determinants of health, epidemiological principles, health equity, and culturally responsive care. Through experiential learning strategies such as community assessments, case analyses, and evidence-based practice activities, students develop competencies necessary to advocate for underserved populations. The course also prepares learners to collaborate effectively within interdisciplinary teams and to design population-focused interventions that improve health outcomes at the community level.
This module concentrates on the expanding responsibilities of Bachelor of Science in Nursing (BSN)–prepared nurses within community health settings. A primary focus is the concept of Social Determinants of Health (SDOH) and their influence on individual and population-level outcomes. Students examine definitions, scope of practice, and real-world applications of community health nursing while engaging in interactive and reflective learning activities that connect theory to practice.
Introduction to Community Health Nursing and Social Determinants of Health
Evaluate how nurses contribute to the improvement of community and population health using evidence-based strategies.
Examine the effects of social determinants of health on health disparities through applied, case-based analysis.
By the conclusion of this module, students will be able to:
Explain the definition, scope, and professional responsibilities of community health nursing.
Recognize major social determinants of health and describe their impact on population health outcomes.
Roles and responsibilities of community health nurses
Overview and implications of social determinants of health

| Task Area | Description |
|---|---|
| Focus | Community health nursing roles and SDOH |
| Instructional Approach | Active learning, case discussions, and applied examples |
| Key Competencies | Population health advocacy, health equity awareness |
This module addresses evidence-based approaches to health promotion, disease prevention, and community assessment. Students explore epidemiological concepts and their application in identifying health trends and managing outbreaks. Emphasis is placed on designing culturally responsive interventions and utilizing data to inform community health decisions.
Health Promotion and Community Assessment
Create health promotion initiatives tailored to the needs of diverse populations.
Apply foundational epidemiological concepts to assess community health concerns.
By the end of this module, students will be able to:
Develop culturally appropriate and evidence-informed health promotion interventions.
Perform a basic community assessment using epidemiological data and surveillance tools.
Principles of health promotion and disease prevention
Community assessment methods and epidemiology
