Part 1: About Me

Answer the following questions to capture how you view yourself in terms of values, aspirations, key qualities, goals, strengths, and areas for growth:

  2. Describe some of your main personal, academic, and professional goals. My personal goal is to achieve better fitness and health. My academic goal is to complete my Bachelor’s degree and potentially pursue a Master’s degree. My professional goal is to become the perioperative manager at my current hospital.

  3. Identify some of your strengths. Consider the top 2-3 abilities you would highlight about yourself, or what others often seek your advice on. These can include course outcomes or other skills and abilities you recognize as strengths. My strengths include being a quick learner, a attentive listener, and a diligent worker.

  4. Mention some areas you would like to improve. I aim to enhance my communication skills, patience, and organizational abilities.

  5. How can these areas help you solve problems and develop a solution-oriented mindset? Improving communication, patience, and organizational skills will enhance my problem-solving capabilities. By gathering all relevant information and considering my actions carefully, I can achieve better outcomes when addressing potential issues.

Part 2: Accomplishment Statements

Accomplishments don’t have to be monumental like scaling Mount Everest or founding a company. Everyday achievements also hold significance. For example, you might have organized volunteers for a school food drive or made a highly successful sale. For those further along in their careers, accomplishments could include promotions or special recognitions.

In this section, you will identify two accomplishments from work, school, volunteer activities, or other areas. You will then practice presenting these accomplishments in a format suitable for a CV or resume.

To begin, review the list of Accomplishment Categories at the end of this template and brainstorm your accomplishments from various activities. After identifying two accomplishments, outline their stories using the information below:

  1. Consider each of your accomplishments in these terms:

    • Challenge: Situation, problem, or requirement.
    • Action: Specific actions you took to address or meet the challenge.
    • Result: The benefit created in specific and measurable terms. Whenever possible, link results to organizational goals.
    • Details: Clarifying details such as numbers or percentages to provide context (e.g., how many, how much, how long, or how often).

    For example, consider a retail manager:

    • Challenge: Increased customer complaints about staff helpfulness.
    • Action: Developed and obtained approval for a new training program for staff and implemented it with a regular review process.
    • Result: Customer complaints decreased after 3 months.
    • Details: Complaints averaged four per week before implementation. After 3 months, complaints averaged one per week or less.
  2. Write two stories using this format (type into the boxes below):

    • Challenge: Managing homework and class assignments.
    • Action: I stay organized by setting a schedule and specific times each day for school work.
    • Result: This organization has allowed me to complete all assignments in a timely manner.
    • Details: Since starting school, I have completed five assignments and one class, staying on track with my degree plan.

    • Challenge: Retained sponges in the abdomen.

    • Action: Implemented the RF wand system.
    • Result: The number of patients with retained sponges decreased.
    • Details: After implementing the RF wand system, the incidence of retained sponges significantly decreased.
  3. Convert your two accomplishment stories into a clear and concise format for a resume or CV, as shown in the examples below:

    Accomplishment 1 Story: I have been able to complete all of my assignments in a timely manner after organizing and scheduling my school work. Since starting school, I am on track with my degree plan.

    Accomplishment 2 Story: Implementing the RF wand system for each patient significantly decreased the number of cases with retained sponges.

Part 3: Reflect on Your Experiences
