Final Preceptor Evaluation

Purpose

The purpose of the Final Preceptor Evaluation is to provide structured feedback on the learner’s overall performance during the practicum. This evaluation serves as an essential tool to document professional growth, clinical skills, and the ability to meet established learning objectives. It highlights areas of strength, identifies opportunities for improvement, and ensures alignment with program and course outcomes. Additionally, this process encourages reflection by both the preceptor and the student, fostering a shared understanding of professional expectations and future development needs.

Instructions

The Final Preceptor Evaluation is administered through Medtrics and is designed for ease of completion. Preceptors receive the form ten days before its due date to allow sufficient time for thoughtful feedback. Once completed, the evaluation becomes accessible to the learner under the “Evaluations” tab.

To successfully complete this assignment, the following steps must be followed:

  1. Access the Evaluation

    • Log in to Medtrics using your credentials.

    • From the left-hand navigation panel, select “Evaluations.”

  2. Locate Your Evaluation

    • Within the My Evaluations section, click “About Me.”

    • Select “View” to open and review the evaluation form.

  3. Review and Comment

    • Examine all sections of the form carefully.

    • Provide any necessary comments to reflect on performance.

  4. Finalize and Sign

    • Scroll to the bottom of the page.

    • Select Submit and complete the process by providing an electronic signature.

  5. Download and Save

    • On the bottom right corner, click “PDF.”

    • Save the completed evaluation to your device for records.

  6. Submit in Canvas

    • Upload the saved PDF into the designated Canvas assignment portal under “Final Preceptor Evaluation.”

Note: This evaluation is not graded, but it is a mandatory requirement for course completion. If students encounter technical issues or require clarification, they are encouraged to contact the course faculty immediately.
